Record the outcome, then the context
For example: 7:10 am · Pee · “Outside after waking; Ben took her.” Choose Accident and its pee or poop type for an indoor accident. Use “No potty” when you offered an outing but your puppy did neither. That keeps an unsuccessful trip from looking like a completed pee or poop.

If a no-potty outing followed by an indoor accident keeps appearing, look at what happened outside and how closely your puppy was supervised afterward. That sequence does not identify a cause. The indoor-accidents guide explains practical next steps and when to call your vet.

Use one row per event. On paper, write the date and add who handled the outing in the note. For a clean printable sheet with more rows, choose “Print a blank log” above when JavaScript is enabled.

Use Pee, Poop, Accident (pee or poop), No potty, Meal, Water, Sleep or Wake. Keep planned trips on your potty schedule and actual outcomes in the log.
Edit or print your entries
Use Edit to change the event, date, time or note. Delete removes one entry; Clear log removes them all after confirmation. Undo reverses your most recent change until the next change or page reload. “Preview & print” prints your entries in date and time order. “Print a blank log” leaves the records out.
Where your log is saved
Entries are saved in this browser profile when storage is available. Anyone using that profile may be able to read them. Clearing site data can remove the saved log. If storage cannot be read or written, the notice above explains that changes last only on the current page—print them before closing or reloading. Browser entries are not shared with your household or transferred to the PupDuty app.
Use notes to explain a change
A record of outings and changes in thirst, comfort or routine can help you explain a concern to your vet. It cannot tell you why an accident happened. VCA’s house-soiling guidance explains why a new change may need a veterinary check.
If your puppy seems unable to pee, seek emergency veterinary care. Contact your vet for blood, pain or difficulty peeing, unusual thirst or a change in frequency. Don’t wait to finish a log. See PDSA’s urine and bladder guidance.
